Applications & Requirements

Funds generated by Choose Life specialty license plate sales and renewals are distributed quarterly to qualifying front-line life-affirming non-profit organizations based on specified criteria.

Each recipient of Choose Life funds first submits an Application along with required documentation. Each center is required to renew it’s application annually via the Renewal Application and documentation.

For Current Recipients:

Renewal applications are due December 1st in order to receive funds the following year. Renewal Application

For New Applicants:

We would love to include you in Choose Life quarterly distributions. Download the Application - New Applicant and the Requirements Check List – New Applicant You become eligible for distributions after approval and the return of your first Quarterly Report.

(Please note that one of the requirements on the checklist is Adoption Training for your front-line personnel. You will participate in a class learning to sensitively and effectively present the option of adoption to your clients/patients. See Adoption Training )

For All Recipients:

Your organization must comply with a non-discrimination policy to help women choose life regardless of race, color, religion, national origin, age, familial status or physical disability and to counsel for adoption as appropriate.

No center that promotes, advocates, refers for, or performs abortions shall be eligible for Choose Life funds.

Funds are distributed in quarterly checks based on the total number of Choose Life Tag sales/renewals, the number in your organization’s county of residence, statistics from your Quarterly Statistical Report. More information here
